Step1
The Initial Call and Care Assessment
On the initial call, we ask questions to form a basic needs assessment. Then we set up an in-home visit with the family – including the family member who may ultimately benefit from our care. This visit allows us to gather even more information, answer questions and let you get a feel for how Ask A Home Care could help you or your loved ones.
The initial interview covers:

Step 2
Custom Care Plan
With Ask A Home Care you will always know what care services are being provided to you or your loved ones. After our initial meeting, we develop a Custom Care Plan. This plan is tailored to a person’s specific needs, and we make detailed recommendations on exactly what kind of services we feel would benefit you. We then arrange a follow-up meeting to go over the Custom Care Plan in detail and modify it as your client sees fit.
Step 3
Caregiver Matching
Once you approved the Custom Care Plan, we will match the most suitable caregiver to execute the care plan. We take numerous factors into consideration when matching the right caregiver with your requirements, such as:
- Care Services Needed
- Your Interests & Objectives
- Personalities of Both the Client And The Caregiver.
- Any of The Client Other Preferences
Step 4
Care Supervision
Our caregivers keep extensive notes about the care they give to you or your loved ones. Those notes are kept at your home and are available at any time. And our billing system provides a detailed description of care services provided, so your loved ones never receives an invoice that doesn’t include a line-item cost of care.
Step 5
Ongoing Follow-up
We have an experienced and dedicated staff that understands how to deliver quality and compassionate care. Our Director of Client Care and Client Service Manager oversees our Caregiver and visits you or your loved one on a regular basis to ensure compliance with the care plan. If there is an emergency or scheduling issue, we have someone on-call 24 hours a day, 7 days a week that can handle the situation. We also have a continuous supply of excellent caregivers who are ready in case your caregiver is ill or has an emergency. And, since all of our caregivers are our employees, we handle their compensation, insurance and benefits. We take care of all the details, so you don’t have to worry about them.
Our Director of Nursing (DON) will begin by performing an initial client Clinical Assessment and develops a tailored Plan of Care (POC) for clients requiring assistance with basic activities of daily living (ADLs). In addition, the DON conducts routine client visits to perform POC updates and service provider supervision.
